Note: If you are apply­ing for Sole Pro­pri­etor Reg­is­tra­tion, you are required to sub­mit to us a Worker’s Com­pen­sa­tion Clear­ance Cer­tifi­cate from the Worker’s Com­pen­sa­tion Board of Indi­ana. We have includ­ed this appli­ca­tion as well, for your convenience.
